Stroke Prevention Clinic

The Calgary Stroke Program’s Stroke Prevention Clinic provides a coordinated approach to stroke care that emphasizes evidence-based practice across the care continuum. People seen at our clinic receive medical investigations to help diagnose stroke and/or identify the cause(s) of their stroke.

Clinic Services

Multidisciplinary clinics run daily and are based at the Foothills Medical Centre. The services provided in the Stroke Prevention Clinic are mainly for clients presenting with symptoms of Transient Ischemic Attack (TIA) or minor stroke; clients require a referral from their family doctor or the emergency department physician (two thirds of patients seen at clinic are referred by their family physician with the remaining third of referrals received from Emergency Departments in the city). Most patients who are discharged from hospital following a stroke are also seen for follow up at the Stroke Prevention Clinic, 10 – 12 weeks after leaving hospital.

Patients at high risk of stroke receive risk factor advice either at the clinic and/or through referral to other agencies such as the Vascular Risk Reduction Clinic, Chronic Disease Management services, the Living with Stroke Program, or Stroke 101 Information sessions. We currently also offer a Lifestyle Education Class on a monthly basis. This session is open to all patients and family members attending the clinic and provides information and advice on stroke risk factors and how to reduce your risk of stroke.

 

Clinic Referral

  • New patients

To refer a new patient to the Stroke Prevention Clinic please fax the referral to 403-944-1154, and include all pertinent medical history, diagnostic test results, and patient contact details. The clinic will contact the patient directly about their appointment date and time.

  • Referral for Discharge Follow-Up

Please fax a discharge summary (including all pertinent medical history, diagnostics test results and patient contact details); the clinic will contact the patient directly about their appointment date and time.
